Educational Trip / Family Emergency Request Form

To request an excused absence, return this completed form to the building principal. For an educational trip, please complete this form at least one week prior to the onset of the trip. Failure to receive prior approval may result in an unexcused absence. A separate form needs to be completed for each child.

Per our Attendance Policy #204 – Absences may not exceed ten (10) school days. Additional days, beyond the 10 per school year, will be considered unexcused.
Trips for students who are not in good academic standing will not be approved.
Trips will not be approved during PSSA, Access and Keystone testing sessions.
Form must be filled out completely.
